תנאי שימוש

Agreement

These Terms of Use apply to your use of lifecareservicesnyc.com (the “Website”), operated by Life Care Services, Inc.. By using the Website you agree to these terms. If you do not agree, please do not use the Website.

Not medical advice — emergencies

The Website provides general information about our home care services. It is not medical advice and does not replace advice from a doctor, nurse or other qualified professional.

In an emergency, call 911. Do not use Website forms or email for urgent or emergency situations.

No care relationship through the Website

Submitting a form, referral or message does not create a patient, client or employment relationship. Services begin only after eligibility review, required assessments and authorizations, and an agreement with our agency. Eligibility for programs such as Medicaid or OPWDD services is decided by those programs.

Information you submit

You agree to provide accurate information and to submit information about another person only if you are authorized to do so (for example, as a parent, guardian or authorized representative). Please do not submit detailed medical records through the Website.

Caregiver resources

Orientation videos, quizzes, training materials and forms on the Website are provided for our caregivers and applicants. Quiz results shown on screen are for guidance; the agency keeps the official training record. Training materials may not be copied or redistributed outside our agency without permission.

• Do not use the Website for any unlawful purpose or to send spam or harmful code.
• Do not attempt to gain unauthorized access to the Website or its systems.
• Do not submit false information or impersonate another person.
