This Privacy Policy explains how Life Care Services, Inc. (“Life Care Services,” “we,” “us”) collects, uses and protects information when you visit lifecareservicesnyc.com (the “Website”) or contact us through it.
This policy covers the Website only. Health information we receive while providing home care services is protected under federal and New York law and is described in our Notice of Privacy Practices.
Information we collect
Information you give us. When you use a form on the Website we receive what you type, such as:
- Contact form: name, email address, phone number and your message.
- Enroll / referral form: client name and address, guardian name, phone number, email, whether we may text you, the client’s diagnosis, the client’s doctor, OPWDD status, care coordination organization and care manager details, and the services you are interested in.
- Caregiver orientation quizzes: name, phone number, your answers and your score.
Information collected automatically. Like most websites, our hosting provider receives technical data when pages load — for example your IP address, browser type, the page requested and the date and time. We do not use advertising or analytics cookies, and we do not sell or rent personal information.
Your browser. The Website remembers the language you choose in your browser’s local storage. This stays on your device and is not sent to us.
How we use information
- To respond to your questions and requests and to contact you about care services.
- To review referrals and determine eligibility for services.
- To process caregiver applications, orientation and training records.
- To send text messages if you agree to receive them (see “Text messages” below).
- To keep the Website secure, working and available, and to comply with legal obligations.
Service providers that process form submissions
To deliver your submission to our office, the Website uses the following third-party services. Each receives the information you submit, under its own privacy terms:
- FormSubmit (formsubmit.co) — relays contact, referral and quiz submissions to our email (info@lifecareservicesnyc.com).
- Google Forms (Google LLC) — keeps a record of referral and quiz submissions.
- Microsoft 365 — hosts our office email.
- Cloudflare — hosts and protects the Website.
Please do not send detailed medical information through the Website. Website forms and email are not intended for detailed health records. If you prefer, call us at (718) 252-1515 and we will collect the information by phone.
Embedded content from other websites
Some pages include content from other companies, which may receive your IP address and set their own cookies when the content loads:
- YouTube (orientation videos) — shown in YouTube’s privacy-enhanced mode.
- Google Maps (our office location on the Contact page).
- Google Fonts (typefaces used to display the Website).
Links to other websites — such as job applications, Google Forms, government training and Google reviews — are governed by those websites’ own policies.
Text messages (SMS)
By providing your phone number to Life Care Services, Inc., you agree and acknowledge that Life Care Services may send text messages to your wireless phone number for any purpose. Message frequency will vary, and message and data rates may apply. For further assistance, reply HELP. You can opt out at any time by replying STOP.
SMS privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with third parties.
How we share information
We do not sell personal information. We share information only:
- with the service providers listed above, so they can perform services for us;
- with care coordination organizations, providers or programs when needed to arrange the services you request;
- when required by law, to protect someone’s safety, or to protect our rights; and
- with your permission.
Security and retention
The Website uses encrypted connections (HTTPS). We take reasonable administrative, technical and physical measures to protect information, but no method of transmission over the internet is completely secure. We keep information as long as needed for the purposes above and as required by law and our record-keeping obligations.
Children
The Website is intended for adults. Parents or legal guardians may submit information about a child who needs care. We do not knowingly collect personal information directly from children under 13.
Your choices
You may ask us to access, correct or delete information you submitted through the Website, or to stop contacting you, by emailing info@lifecareservicesnyc.com or calling (718) 252-1515. Reply STOP to any text message to stop texts.

We may update this policy. The date at the top of this page shows when it was last updated.
